Representation / Agency review first
Broker of Record Review
Ask Kalvira to review becoming your broker for one specifically identified policy. Start with the facts; complete any appointment only after agency and carrier review.
Use your personal or work email. Review, save a draft, and keep your submitted copy in your private account.
What happens next
- 01
Kalvira checks the licensed entity, carrier access and policy eligibility.
- 02
The agency confirms scope, timing, required disclosures and the carrier's accepted appointment document.
- 03
An authorized insured chooses whether to sign the reviewed appointment. Carrier confirmation is separate.
Representation is not replacement coverage.
A reviewed BOR appointment may change who represents you on a policy when accepted under the carrier's rules. Switching insurance companies requires a separate insurance application and coverage decision. A loss-run authorization only requests loss history; it does not change your broker.
This preparation tool is not an official carrier form or ACORD 36. Carrier requirements and agency/legal review come first. Only need loss history? Use the loss-run authorization.
Reference: State Fund's BOR process illustrates carrier-specific eligibility and acceptance rules; it is not a statement that Kalvira is appointed with State Fund.
No automatic carrier submission, coverage change, cancellation, certificate issuance, or broker appointment occurs through these request forms. Do not use them for claim reporting or urgent coverage needs.
